C100 – To apply for contact, residence, specific issue, prohibited steps applications. This cannot be issued without attempting mediation first unless one of the legal exemptions applies such as domestic violence.
CB2 – Urgent hearing without notice for Child Arrangements Order. This can be used when a parent fails to return a child.
C2 – To start proceedings or ask for an application within existing court proceedings as well as if you do not hold parental responsibility (e.g. grandparent) or are subject to a barring order or to request something, such as changing a child’s name.
C1a – Allegations of harm and domestic violence, usually completed at the same time as C100.
C79 – To enforce a child arrangements’ order, for example if the father is not keeping to contact arrangements or if he has failed to return the child from contact.
C8 – To apply to keep your contact details confidential from other parties.
N161 – To appeal a court’s decision. You need legal ’grounds‘ to do this, not just that you do not agree with the outcome.
EX160 – To help paying the court fee if on benefits or low income – this can be used with any family court applications.
D151 – To apply for a judge hearing family case to include a maintenance order.

C1 – To make an application regarding a care order or associated applications such as breaches in any court orders.

C1a – Allegations of harm and domestic violence. This form can be submitted to the judge as supplemental information when you are in public law proceedings and your domestic violence is not being taken seriously.
